Registration Form (click here for the registration form)
Please complete this form and mail, fax or deliver it to the office. Child must be 4 years old by July 31, 2008 to participate in the fall 2008 season. If this is the first season your child has played with SSA, please include a copy of his or her birth certificate. If the form is incomplete or incorrect payment was sent, SSA will return the form to you for correction.
Medical Waiver
Please complete this form and return it to your coach. SSA does not need this form returned to the office. ALL COACHES MUST HAVE MEDICAL WAIVERS IN THEIR POSSESSION DURING PRACTICES AND GAMES.
Application to Play Up
Please see Policy Statement 3. Required for any player who wishes to play on a team older than his or her age group in the Recreational Division. (Restrictions Apply)
Refund/Reimbursement Form
Form needed to request a refund of registration fees (prior to start of season) or other such fees. Also to request reimbursement of money spent on behalf of the association.
Application for use of SSA properties
Form required to schedule field usage for matches.
Application for Voting Membership
To qualify to vote at the annual meeting of SSA, this form must be completed.
Accident Insurance Claim
KSYSA provides supplemental insurance for players. In the case of an acccident during practice or a game, please see this link.
