Forms for Referees
Referees at Sunflower Soccer Association are independent contractors, hired by SSA, but assigned games based on age, skill, experience, game level,
and availability by an assignor, another independent contractor. Referees are available for hire if currently registered with the United States Soccer Federation.
Please complete the referee service agreement, spring 2010 availability and W-9.

Referee Service Agreement
This form is filled out once per year. If you completed it in the fall, there is no need to fill out another. Please read carefully. There is a fine for 'no-show' matches or matches that you do not find a replacement for that you are unable to officiate.
W-9
Referees are contract employees based on the referee service agreement. If you make $600 or more per year, SSA will provide you with a 1099 for tax purposes. Please note, as contract employees SSA does not pay social security or income tax for you. This form only needs to be completed once per year.
Coach Ejection Incident Form
In the case of a coach ejection from a game, the center referee must complete this form, within 24 hours and submit it to both Sunflower Soccer Association and the Kansas Youth Soccer Association.
